CASE scenario....

Hi, I am k. Prudvipriya, 3rd year bds student. This is an online elog book to discuss our patients health data after taking her consent. This also reflects my patient centered online learning portfolio.

CASE HISTORY : A 60year old known case of suffering from typhoid 
CHIEF COMPLAINTS : 15 days associated with weakness and pain at abdomen, vomiting 

HISTORY OF PRESENT ILLNESS : 
Loss of appetite since 2 months and fever 
Relieved by taking medication, burning micturition


PAST ILLNESS :
Epilepsy since 4 yrs and also hypothyroidism from 4 yrs

PERSONAL HISTORY : Veg deit , Loss of apatite, have proper sleep, regular bowel, tobacco-yes

FAMILY HISTORY: no history of similar complaints in the family 

GENERAL EXAMINATION:
Patient was moderately built and moderately nourished
Pallor-yes
Icterus-yes
Cyanosis -normal
Clubbing - no clubbing 
 Edema -pitting edema till knee
 
VITALS
Temperature -99°c/f
Pulse-96 beats per minute
Bp-130/70mm of hg
Spo2-98%
