Swelling of legs, difficulty in breathing from 2 days

Patient was asymptomatic 10 years back then there was onset of back pain and swelling of legs-bilateral  pitting type 3 days ago difficulty in breathing, and 
Relief on sitting 
Aggressive while sleeping 
History of past illness
Hypertension since 8 yrs

Patient was moderately built and moderately nourished

Appetite-loss of appetite
