CASE scenario....

Hi, I am k. Prudvipriya, 3rd year bds student. This is an online elog book to discuss our patients health data after taking her consent. This also reflects my patient centered online learning portfolio.

CASE HISTORY : A 43year old known case of chronic renal failure
CHIEF COMPLAINTS : swelling of lower limb from 15 days

HISTORY OF PRESENT ILLNESS : 
Patient was apparently asymptomatic 2 years ago,then he developed pain and swelling of lowerlimb .he's been taking treatment from 6 days.relieved on taking rest pain was not suspended on taking painkillers 


PAST ILLNESS :
Treated with painkillers injection 
Generalised weakness 
Low appetite 

PERSONAL HISTORY : 
Mixed diet, Loss of apatite, have proper sleep, regular bowel, no habits 

FAMILY HISTORY: no history of similar complaints in the family 

GENERAL EXAMINATION:
Patient was moderately built and moderately nourished
Pallor-normal
Icterus-normal
Cyanosis -normal
Clubbing - no clubbing 
 Edema -no pitting edema
 
VITALS
Temperature -98°c/f
Pulse-96 beats per minute
Bp-160/80mm of hg
Spo2-98%
