 Hello, I am kamuni prudvipriya 3rd year dental student. This is an online elog book to discuss our patients health data after taking his/her consent. This also reflects my patient centered online learning portfolio.

A 64yr old male came with 
Chief complaint of chest pain since 1month and difficulty in breathing since 1 month ,
 cough since 7days,
 neck pain since 7days and fever since 7days

History of the present illness
Patient was apparently asymptomatic 1month ago the he developed chest pain which is on left side and radiating to upper limb and back of the neck
No relation to food intake burning and spasmodic type of pain and shortness of breath since 1month and gradually progressed  from grad 1to grade 2
Cough is productive white foamy and sob is more on cough and in supine position
Neckpain since 7days radiating pain 
B/L shoulders  and difficulty moving head side to side
Fever since 7days high grade not associated with chills no duration variation and seasonal variation

History of past illness -similar complaints of SOB 2months back relived on medication
Similar complaints of chest pain 2months back relived on medication
Generalised weakness of legs while walking
Personal history -diet -vegetarian
